Schedule:

00 am - 5:
00 pm, Monday to Friday.
Patient Panel:
You'll be responsible for a patient panel of 300 individuals.
Patient Load:
Expect a manageable 5-10 patients per day, with 1-hour appointment windows.
Population Health Management:
Utilize a proactive approach to monitor and manage patients' disease processes, reducing ER visits, hospital admissions, and readmissions.
Collaborative Team:
Work closely with an integrated team comprising social work, care management, behavioral health, pharmacy, and more to provide top-quality care to vulnerable patients.
Transitional Care Plans:
Develop clear transitional care plans for your empaneled patients.
Compensation:
Your compensation package will be tailored to your experience, with value-based care and FQHC experience being preferred.
Experience Requirement:
A strong background in outpatient primary care is a must, with at least 5 years of experience.
Compensation Package:
Competitive pay based on experience plus an annual bonus of 20%.
The practice currently consists of one physician, one APP, and two NPs.
