Clinical Director- Skilled CareThe Clinical Director (Skilled Care) is responsible for the organization and direction of the clinical service program.
The position participates in all activities relevant to professional and paraprofessional services.
QUALIFICATIONS Registered professional nurse with a current valid Pennsylvania license.
Five (5) years of current experience as a registered nurse with roles reflective of progressive responsibility, including supervision.
Home care experience preferred.
Graduate of a state-approved RN School of Nursing.
Baccalaureate degree preferred.
Interpersonal, communication, and organizational skills.
Evidence of leadership activities.
Holds a current CPR card.
Current driver with a valid driver's license, car insurance, and registration, and a reliable vehicle.
Drivers attain and retain an acceptable motor vehicle record.
Obtains and retains a clear criminal history report, Childline, FBI, OIG, Medicheck, EPLS/SAM and any additional required regulatory clearances.
Knowledgeable in the OASIS Comprehensive Assessment (The Outcome and Assessment Information Set) and Medicare's PDGM (Patient Driven Groupings Model).
CLINICAL/TECHNICAL SKILLS Provides leadership in promoting and maintaining standards for the delivery of high-quality care by all members of the care team.
Ensures that each patient/consumer receives quality-based, appropriate service by monitoring staff assignment, clinical review, and coordination of care by all disciplines.
Assists staff in developing, implementing, and evaluating care plans for patients/consumers, utilizing a supervisory and developmental role.
Conducts care-review conferences with care managers.
Assists in the hiring, evaluating, and terminating clinical employees, maintaining qualified staff in sufficient numbers to serve patient needs.
Plans for and oversees all staff education, including the orientation of new staff, performance appraisals, and yearly competency evaluation of all professional and paraprofessional.
Assists with receiving patient care referrals, consulting with physicians for orders and planning of care development.
Participates in and ensures 24/7 on-call access for patients/consumers and staff.
Maintains clinical skills by providing hands-on care to clinically complex patients and assisting with patient care visits and on-call, as necessary.
Applies problem-solving abilities to specific organization/clinical activities.
Coordinates and implements employee health testing such as TB, hepatitis B vaccine per policies.
Works with the Clinical Director and the Administrator to ensure quality improvement and regulatory and certification compliance at local, state, and federal levels.
Works with the Clinical Director in establishing and reviewing clinical functions cooperatively with the Professional Advisory Committee and the governing body.
DOCUMENTATION Responsible for the training of new staff on the agency's software system to ensure consistency throughout the continuum amongst all disciplines.
Reviews the patient record for completeness, appropriateness, and accuracy, verifying and validating the comprehensive assessment, OASIS information, coding, Patient Driven Group Model (PDGM) as applicable based on requirements.
Reviews and updates policies and procedures, reporting tools, and clinical record forms, annually and as needed.
Works with the Compliance Manager to ensure that clinical records are current and in accordance with the agency's policies and regulatory bodies.
Directs the clinical record system including the clinical aspects of the billing process.
Plans for, oversees, and/or implements the agency's clinical record review process.
Responsible for the agencies Quality Assurance and Performance Improvement (QAPI) program, and updates annually and as needed.
COMMUNICATIONS Ensures the confidentiality of clinical communication, including written, verbal, and computer information all within HIPAA regulations.
Assures the coordinated communication of clinically oriented agency priorities, functions, and directives to all constituent groups.
Represents the agency on clinical matters in the community.
Organizes and implements staff meetings.
Maintains an open-door policy to resolve patient and staff concernsPROFESSIONAL STANDARDS Maintains a professional standard of conduct, always projecting a positive image of the agency.
Complies with infection control, risk management, and safety policies and procedures.
Adheres to all policies and practices of the agency to maintain established Standards for Care.
Adheres to all policies set forth in the Nurse Practice Act and the nursing practice standards to maintain safe clinical practice.
PROFESSIONAL DEVELOPMENT Continually strives to improve nursing care by broadening and deepening knowledge through formal education, attendance at workshops, conferences, active participation in professional and related organizations and individual study and reading.
Attends mandatory in-service education annually.
Presents in-service education sessions for the staff.
Seeks clinical collaboration in new and/or unfamiliar care areas.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
